Flexor tenosynovitis from M. tuberculosis concomitant with median nerve involvement has been rarely reported, and it usually presents with symptoms of carpal tunnel syndrome (CTS). In this report, we introduced a patient with symptoms of CTS with rice body in the volar wrist synovium membrane and extrapulmonary tuberculosis (TB). Rice bodies were presented in synovium and histological study indicated the inflammatory granulomatous with necrosis caseous. Surgical debridement with excision of the involved synovium and irrigation was performed.
